Preparing for Your First Visit

  • Alternatively, arrive 15–20 minutes early to complete the forms in our office. This ensures a smooth start to your child's healthcare journey with us.

What to Bring on Your First Visit

  • Immunization record(s)
  • Current insurance card(s)
  • Parent/legal guardian photo ID
  • Court documents (if pertinent)
  • A copy of the hearing screen (for patients less than 1 year old)
